Notice of Privacy Practices
Effective Date: 10/2/2026
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Our commitment to your privacy
Compass Recovery, Inc. is required by law to maintain the privacy of your protected health information (PHI), to give you this notice of our legal duties and privacy practices, to follow the terms of the notice currently in effect, and to notify you if a breach affects your unsecured PHI. Because we provide substance use disorder (SUD) treatment, your records are also protected by federal confidentiality rules at 42 CFR Part 2 and by California law, including the Confidentiality of Medical Information Act.
How we may use and disclose your information
- Treatment: to provide, coordinate, and manage your care among our clinical team and, with your consent, with outside providers.
- Payment: to bill and collect from your health plan or other payers, with your consent where Part 2 requires it.
- Health care operations: for quality improvement, staff training, accreditation, licensing, and business management.
- With your written consent: Part 2 generally requires your written consent before we disclose SUD records. You may give a single consent covering future uses for treatment, payment, and health care operations, and you may revoke it in writing at any time, except for action already taken.
- Without your consent, only as the law permits: medical emergencies, reporting suspected child abuse or neglect, certain research and audit activities, public health reporting of de-identified information, and a court order that meets Part 2 requirements.
Special protections for SUD records
Your SUD treatment records, or testimony describing them, may not be used or disclosed in any civil, criminal, administrative, or legislative proceeding against you unless you give specific written consent or a court issues an order that meets 42 CFR Part 2. Federal law restricts anyone who receives your records with your consent from redisclosing them except as permitted.
